This will streamline your visit saving you time upon your arrival. These forms will help us get to know you and help us understand your pertinent health information, allowing us to provide you with the most effective care possible. Insurance and Payment

Insurance

Here at Intermountain Dental, we are committed to providing you and your family with quality, comfortable dental care. Our team will work to help you receive the best treatment possible and to find the treatment option that best fits your financial situation. We welcome almost all types of dental insurance and we are preferred providers for Blue Cross of Idaho and Delta Dental.
PLEASE UNDERSTAND that we file dental insurance claims as a courtesy to our patients. Insurance plans are a contract between you and your insurance company and therefore, we cannot be responsible or guarantee how your insurance company will handle claims or payment made by them. We will, however, always do our best to assist you by estimating your portion of the cost of treatment and helping you navigate through this process. Most policies are clear and specific in which procedures are covered and what portion of the cost you will be responsible for and our team here at Intermountain Dental is ready to assist you and ensure you receive maximum reimbursement. We will also provide your insurance company with x-rays and reports if requested by your insurance company.

If there is any remaining balance on the account after insurance pays on the submitted claim, that amount is due in full the following billing period. Partial payments are not permitted without prior agreement by this office. If the insurance company sends you the check, you are responsible for remitting that check to Intermountain Dental, along with any balance. As a courtesy, we do not charge you up front for your insurance company’s portion.

If your insurance company denies a claim, you are responsible for remittance of the balance in full. Payment arrangements for account balances are due at the time services are rendered, so please bring your insurance card with you to your first visit.

Payment

For your convenience we accept Visa, Mastercard, Discover and American Express. We offer a 5% discount for cash payments.
We do not offer any in-office payment plans; however, we are proud to offer financing options through Care Credit. Ask our front office staff if you have any questions regarding these payment options.
